Appointments: We see patients by appointment only and do offer same day where availability permits. We ask that each patient arrive 15 minutes prior to appointment to update any chart information and provide paperwork if applicable to the appointment. 

Insurance Cards & Identification: All patients utilizing their insurance coverage are required to bring current information with their photo ID or drivers license to each appointment to limit billing complications. Please contact our office for more information on plans we participate with.

Copays, Products & Self-Pay: Payment is to be made at the time of service and while cash is accepted we do not provide change and recommend using a debit/credit card. We accept VISA, Discover, Master Card & American Express. Cosmetic services can be paid by care credit if the patient requests. Our current self-pay fee is $200 for each visit but subject to change according to services provided. Please note if your visit requires a biopsy the self-pay fee from the lab is not associated to our office and pricing will be dictated by them. 

Minors: All patients under the age of 18 years old will need a legal guardian chaperone i.e. parent(s), unless agreement is made and documented between MD Dermatology and patients representee.  If guardian is not available at time of appointment, we can reschedule it to a future date. 

Referrals & Authorizations: If an insurance plan requires a referral to be seen by a specialist; it is the patients responsibility to obtain one from their primary care physician, etc. We ask you have the referral prior to scheduling an appointment, you may contact us for our fax number you can provide to your doctor to have them fax it to our office for your convivence.
