Insurance cards and Identification

 All patients utilizing their insurance coverage are required to bring their current insurance cards and your driver’s license or photo ID. To know if your insurance is accepted, please contact us!

Copays, products or self pay

Payment is to be made at time of service and may be made by cash, check, VISA, Discover, American Express or MasterCard. For cosmetic services we do accept Brilliant Distinctions, Care Credit and Aspir

Appointment Time

Patients please arrive at least 15 minutes prior to your scheduled appointment time. At your visit, you will need to allow yourself enough time to check in, have your ID scanned into the system and complete any paperwork.

Referrals & Authorizations

 If insurance referrals and/or authorizations are required, it is the patient’s and primary care physician’s responsibility to provide that authorization. We must have the referral prior to scheduling your appointment.


ALL MINORS must be accompanied by a parent or legal guardian to sign the appropriate consent form and speak with the provider about treatment options. If the parent or guardian is not available, we can reschedule the appointment. We are unable to know the financial responsibilities of divorced parents, so we will look to the adult accompanying the minor for payment.

Our Policies


Please contact our office at (410) 956-7777 (Edgewater), (410) 451-0700 (Waugh Chapel), or (301) 863-7310 (Lexington Park) as soon as possible to cancel or reschedule your appointment. Please provide at least 24 business hours notice. We will attempt to contact you by phone with a courtesy reminder a few days prior to your appointment.

No Show/Late Cancellation Policy
A No Show fee will be charged to your account of $50 if:


§ You do not show up for your scheduled appointment.

§ You do not cancel your scheduled appointment at least 24 business hours notice prior to your scheduled appointment time. This includes all established appointments, medical procedures and cosmetic appointments and treatments. Please note: we require at least a 7 day notice of cancelation for all surgical appointments.

If you do not provide 24 business hour notice of cancellation for cosmetic appointments your deposit will be forfeited.
